brown roots

my plants have brown roots i put new water in my tank but the brown root came back what can i do to stop this.

Is the plant wilting at all? A brownish color is generally the color of roots anyway so what is different about this?

Did the roots ever dry out? Has pH been checked and is the proper amount of fertilizer being used?
